You may use this form to return your completed sweepstakes report to Debra Sandoval but before doing so please be sure that you have received verification of your issued sanctions from Deb. Be sure to fill in all the blank spaces at the top of the form including both the ARBA number and the Holland Lop sanction number issued to you for your show. A separate exhibitor list must be included for each show. Return form either to Debra Sandoval at the address at the bottom of the form or by email to: debra.sandoval@gmail.com.
